Creating a form in Powerpoint? - Microsoft Community

i instructional designer @ university of st. francis, joliet, illinois, , trying nursing instructor project.  wants create powerpoint explains elements of well-written prescription (for medications).  

at end of powerpoint, wants insert form prescription form, fields patient's name, date of birth, address, , description number of refills; , other data.  (see image).  then, after student completes form, instructor receive information.  

is there microsoft app or app microsoft recommends used create form?  

preciousleb's answer correct far goes.  it's true powerpoint wasn't designed type of thing, there couple ways of approaching problem want solve.

the simplest would, think, create fillable form in other program, link @ end of ppt show.

but if want make entirely self-contained within 1 powerpoint show, i'd have @ david marcovitz' site.  there lots of examples there, , may decide book worthy purchase:
