Need a help to create template in word 2010 - Microsoft Community

hi,

can please me create template?

i'd tried lot different combination still couldn't found solution create template below snapshot

please see below image

image 1:

1                          - chapter no , should increment when new chapter created.
chapter title      - title of chapter (heading 1)
1.1 introduction - sub chapter title (heading 2)
book title           - book title of document should automatically come cover of doc.
rev 1.0                - revision no. feild placed in cover page , should reflect in footer shown

image 2:

abc company    - company name
chapter title      - chapter title should automatically refer page new chapter created. in case refer image 1
1.1 introduction - sub chapter title.

abc company & chapter title: same mention in image 2

image 4:

abc company & chapter title: same mention in image 2

if compare image 2,3 , 4. sub title (below chapter title) referred first sub title (heading 2) of page

or
if there no sub title (heading 2) created on page last created sub title (heading 2) reffered.

after doing table of contents format should remain same there in sample (refer below easy reference)

1 chapter title----------------------------------------------------- 5
  1.1         introduction------------------------------------------------ 5
  1.2 observation------------------------------------------------ 7
  1.3 test report------------------------------------------------- 7
  1.4 conclusion------------------------------------------------------- 8
