Word Fill in Form Field (Link) - Microsoft Community

hi 

how can design form field in word links content of fill in form?

for example, have form same persons name has entered in 2 places in form

so instead of inserting 2 text fill in form fields have type persons name twice when filling out form, type name once in first field , have second field link name appears there automatically when enter name in first field.
